A hat you can put an extra consumable into (a medkit, a grenade, or some other consumable that gets added in the future). Taking a consumable out of a hat should take some time, so item use binds should use normally carried consumables first. If you have a grenade in your hat and a tyrant crushes you, the grenade should explode and damage the tyrant.
Why not just give the player the ability to just hold an extra item slot?

Why not just give the player extra ammo instead of a battpack? Besides, basliks could be able to steal the hats.

Because giving players a shitload of ammo beyond normal as default isn’t a good way to deal with power creep. Then again neither was Stage 1-3.
